Payment Policy
Quartz Mind, Body + Skin Clinic Inc. and the Practitioners of Dr. Paaras Kaur ND Inc., who rely on Quartz Mind Body & Skin Inc. for administration purposes require a credit card on file for contactless payments. This helps keep both you and us safe, as well as spend more time with you, rather than taking a payment after your session. At the end of your visit, you will be responsible for full payment for all fees for the treatments provided to you by Quartz Mind Body & Skin Clinic Inc. and the Practitioners of Dr. Paaras Kaur ND Inc.
Furthermore, the Practitioners of Dr. Paaras Kaur ND Inc., who rely on Quartz Mind Body & Skin Inc. for administration purposes, does not collect payment from MSP. If you are on income assistance and will be applying for reimbursement from MSP then a claim form will be filled out and a copy will be given for you to submit. If you are claiming reimbursement from an extended medical plan then the standard receipt that you will be supplied should be sufficient for your provider to process your reimbursement.

Cancelation & No-Show Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the technicians’ or Practitioners’ day that could have been filled by another patient. As such, each patient of the Practitioners of Dr. Paaras Kaur ND Inc. being treated at Quartz Mind Body & Skin Clinic Inc. and each client of Quartz Mind, Body & Skin Clinic must provide at least 24 hours notice if their appointment needs to be canceled or rescheduled. Notice of cancellation or rescheduling must be given to Quartz Mind Body & Skin Clinic during its clinic hours or prior to its regular clinic hours covering this 24 hour period. If you fail to cancel or reschedule within 24 hours, you will be charged the full amount of the treatment and, if applicable, the cost of non-reusable products prepared for that treatment.
Treating Minors Policy
For minors, being individuals under the age of 19, an in-person signature from their parent (who has not stopped being their guardian) or guardian confirming they are aware the minor will be receiving treatments from Quartz Mind Body & Skin Clinic Inc. and/or Dr. Paaras Kaur ND, Inc. will be required. For minors under the age of 16, in addition to this in-person signature, their parent (who has not stopped being their guardian) or guardian must be physically present while they are being treated.
Photography
During your consultation and at various stages throughout your treatment(s), the technicians of Quartz Mind Body & Skin Clinic Inc. and/or the Practitioners of Dr. Paaras Kaur ND, Inc. will ask to photograph you. The photographs being taken of you to observe progression, evaluate treatment effectiveness, training, professional publications, sales and/or documenting results. They will never be shared without your consent.
Informed Consent Policy
Scheduling an appointment at Quartz Mind, Body & Skin Clinic for a specific treatment will be considered consent to that treatment. Prior to receiving the treatment, the technicians of Quartz Mind Body & Skin Clinic Inc. will review the possible risks, benefits, costs and adverse effects of the treatment with you.
As a diagnosis is made and treatment options are presented to you, the practitioners of Dr. Paaras Kaur ND Inc., will ensure you are informed of the possible risks, benefits, costs and adverse effects of the proposed treatment and any alternate treatments, along with any risks of not treating the diagnosed condition. If you choose to proceed, you will have to sign a consent form and/or verbally agree to the proposed treatment. Scheduling an appointment for a specific treatment will be considered consent to that treatment.Â
